I noticed that many of my quizzes that have html embedded images are unable to visualize them. Could it be a site wide issue?

Unfortunately, you're going to have to manually fix this.

From time to time we remove unused images. If you hack a quiz together by putting in an image using html then we don't know about that.

The workaround is to create a picture quiz called "image storage" or something and then use the images from that quiz.
