How to insert a click quiz into an image ?

I've seen quizzes using this feature, like the Disney characters Click Quiz and find it really excinting. My idea is to make it more interesting by creating a "School of Athens" (the painting by Raphael depicting all the famous philosophers from Antiquity) quiz. The name of the philosophers would appear and the user would simply click on the painting ! This feature, instead of simple image cutting with text, would really preserve the core of the painting. But I don't know how to do it.
