How to Make a 'Groups of Things' Quiz

I wanted to make a groups of four quiz for a certain theme, and wanted it to be formatted like this: Groups of four quiz, but I'm not sure how to do it. Is it through using the manual mode in design mode, or is there like a button to do that (I feel like I remember seeing one before for some reason).
