Issues With Picture Quiz

Can anyone help? I am trying to create a picture quiz, but every time I write an answer, it changes all the answers to the one just written.

Checked through all the quiz settings, but have no idea how to fix this.

Does anyone have a solution please?

It should be fixed. Unfortunately, any quizzes with problems are permanently burned so you'll have to create a new quiz and transfer the images from the old quiz.
