Pneumonia is a bit unclear since it can be caused by many different pathogens. There are vaccines against the most important ones, such as against Haemophilus influenzae and Streptococcus pneumonia, however not against every potential one.
Lyme disease is absolutely preventable by vaccine, there is no commercial Lyme disease vaccine on the market currently in the United States, but there used to be a Lyme vaccine (Lymerix) on the market which was withdrawn from production because it wasn't profitable, not because it didn't work. There are also Lyme disease vaccines on the market for dogs.
I've changed it since it is theoretically preventable. I will note that Lymerix had an efficacy under 80% so perhaps that contributed to it being unprofitable.
