In a world where Ryanair calls an airport 70km away one that is serving a particular city, obviously there could be arguments that many of these have valid alternatives. However, Milan (LIN) and Moscow (DME) are perhaps significant enough to be worth acknowledging, in the way you have with Paris-Orly and London-Gatwick.
