IMO including small island countries kind of defeats the interest. Not having Bangkok or Addis Ababa tells me the methodology for inclusion has to be changed

I think that excluding some cities just because their countries are "too small" feels a bit arbitrary. They still fit the description of primate cities regardless of how small the countries are, and including certain cities just because they seem like a good fit sort of defeats the purpose of the quiz. The two that you mentioned aren't even in the top 5 of their respective continents, even if you ignore all primate cities with a population of less than 1 million.
