Does Georgia Tennant in the Doctor's Daughter not count? I guess she is a bit more like a side character.

The caveat states that it only includes actors whose names have been featured in the opening title sequence, which she wasn't. That's why I've not put her as a correct answer.

You also state that it is people who played the doctor or a companion. John Simm's master was never a companion and even if they are in the title credits Nick Frost, and Kylie Minogue were never companions either.

Kylie Minogue definitely was a companion, just as David Morrissey, Michelle Ryan and Lindsay Duncan were in their episodes.
