I think you need to adjust the colors on your TV, if you think Ki-Adi Mundi is orange and Shaak Ti is blue.

Take Shaak Ti out she defintitely doesnt desreve to be classed as 'famous' and Ki-Adi-Mundi's description is very misleading as it says he has orange skin,that is the probably the description for Plo Koon please fix this.Also,replace Shaak-Ti with Kit Fisto,much better jedi to be putting into a quiz.Other than that very entertaining quiz.

Yes but anakin didn't become Vader until the end of revenge of the sith, so he is only in episodes 3-7

Anakin and Vader are the same character. They may look or feel different but they are still the same person. Anakin was never loyal to the order or the republic, he was loyal to the people in these organizations. Palpatine twists Anakins view of those people to further guide him to himself.

After adopting the name Vader and doing all these abhorent things he cant forgive himself and thinks no one could ever forgive him. He stays loyal to Palpatine because he's afraid of him.

Only after Luke (the very last piece of Padme, THE love of his life) forgives him he finds the courage to stand up to Palpatine because he's loyal to Luke, as he was loyal to Padme.

Anakin is Vader and Vader is Anakin

Though this may be purely philosophical conjecture, I believe it wasn't Darth Vader who saved Luke Skywalker by killing Palpatine but Anakin Skywalker redeemed.
