Technically speaking, both October and November Revolution would be correct due to the difference in calendar. Also, there was a March Revolution too, so it would've been better if you mention the second revolution.

3. The correct answer for ruler is the Emperor, not the Tsar. Russia was the Empire since 1721. You can check wikipedia for that matter. I can agree that you can use both of these titles, but it doesn't really makes sense to state the Tsar as the only correct answer.
4&5. Can you elaborate, please? There were three revolutions in Russia: First Russian revolution (1905), February revolution (1917), October Revolution (1917). I get that all answers lead to the only right answer, but it's still wrong to just say the "Russian Revolution" when there were three of them.

No one says in Russia "November" or "March" Revolution. It is deeply ingrained in culture to name these events after the days in Julian calendar.
