Teams That Have Won the English League, FA Cup and a European Trophy

Twelve football clubs have been champions of England, won the FA Cup and lifted a major European trophy during their respective histories. How many can you name?
The seasons given are those in which each team became eligible for this list by winning the third different type of honour.
The European competitions included here are the European Cup/UEFA Champions League, UEFA Cup/UEFA Europa League, its non-UEFA predecessor the Inter-Cities Fairs Cup, the European/UEFA Cup Winners' Cup and the recently formed UEFA Europa Conference League.
