Including El Alto is a bit tough, cos it's just a suburb of La Paz.

San Juan should be called "Pasto", nobody calls it San Juan. The full name is San Juan de Pasto, just like Bogotá really is Santa Fe de Bogotá and Cali is Santiago de Cali. In fact, there are other places called only San Juan in Colombia, which makes it even more misleading.
