At no point in the books is alchemy mentioned as a subject at Hogwarts. Why is it included in this quiz?

Agreed. Either expand the instructions past just the books or remove that answer, there's no way to get that just by reading the books (or even watching the movies I think).

Alchemy is only mentioned as a course on Pottermore. Not in the books. According to Pottermore, Alchemy is offered to sixth and seventh years only. No requirement for the class is mentioned though I imagine you would need an OWL in both Potions and Transfiguration. 🧪

Elective just means it's not mandatory. You still get exams for elective courses.
