First, I'd say Vader is in fact number two within the Empire. It's out of respect for Tarkin that he listened to him in A New Hope, not because he outranks him.
As for the Wedge Antilles question, it's sort of a frustrating trap... of course he only participated in battles during the OT, it's obvious we don't expect him to fight when he wasn't born. Ask anyone the same question, they'll just assume you're talking about the OT only.
As I said though, good quizz, I had fun doing it.
As for your second point, I do lots of trick questions on purpose, so read carefully! :)
